The LA Wallet app now lets you access all your dependent's Louisiana vaccinations, not just COVID-19.

In order to link their vaccination(s), tap the Vaccine Card on the Home tab and tap the Add Dependent Vaccination Card button at the bottom of the page. Fill out the boxes with your child/dependent's information, and tap Add Dependent. When their info is found, it will automatically appear.
- Tap their name, then Vaccine Card at the top to see their immunization history. To view only COVID-19 vaccinations, tap SMART Health Card.
If not found or they're missing a vaccine, please open the Menu tab in the LA Wallet app and then tap on Vaccine Card Assistance. That will get you a ticket opened in a support channel with the Louisiana Department of Health (LDH) who will be able to assist further with any vaccine related issues.
NOTICE: Anyone 18 years or old is not considered a dependent and will need to create their own LA Wallet account to show their vaccine information.
If you do not see this section, please make sure you have the latest version of the app installed.
