In order to link your Covid vaccination to LA Wallet you must first create an LA Wallet account and link your Louisiana driver's license or state ID to that account. Next, tap the SMART Health Card on the Home tab and tap Retrieve SMART Health Card. Once your information is found, tap Yes to add your SMART Health Card to your account.

If not found, please open the Menu tab in the LA Wallet app and then tap on Health Card Assistance. That will get you a ticket opened in a support channel with the Louisiana Department of Health who will be able to assist further with any COVID-19 related issues.
