- The information is sent and the issue will be handled directly by the Louisiana Department of Health (LDH).
Instructions
1. Tap the Menu tab, scroll under Louisiana State Services, and select the Vaccination Card Assistance option.
2. When asked if you need help with a health card, select My Child/Dependent's Health Card.
3. When asked about your issue, select My child/dependent's health card was not found or is incorrect.
4. When asked if the child/dependent's vaccinations were given outside Louisiana, select No.
5. You will be asked if the child/dependent's vaccinations were given at the Veteran's Administration (VA). Tap Yes.
7. You will be asked if you've contacted the VA for an Authorization to Release Protected Health Information form.
- If you select No, you will be informed to contact the VA to get this done then check later for the vaccination card once it's been completed.
- If you select Yes, you will need to fill out a form for additional assistance. Once all boxes with a red asterisk(*) have been filled out, tap Next to go to the next page.
- You can provide a phone number for LDH to contact you about the vaccinations.
8. The next page will require you to Select Reason for Escalation and submit a picture then tap Submit for a ticket to be escalated to LDH.
7. LDH will handle contacting and assisting you from here on out. You will need to reply directly to any email or phone communication that you receive from LDH.
