Key Information

• LA Wallet supports Medicaid cards for United Healthcare, Healthy Blue, Louisiana Healthcare Connections, AmeriHealth, Humana, Aetna, and Fee-for-Service health plans.
  • You cannot add a Medicare card as LA Wallet does not support Medicare, only Medicaid.
• If you can't find your Medicaid Health Plan Card using your SSN or CCN, email help@lawallet.com with the following details to escalate the issue to the Louisiana Department of Health (LDH) for further help.

Instructions

1. After linking your valid driver's license or state ID, tap the Medicaid Health Plan Card on the Home tab.

2. You will see a Terms and Conditions page which you can accept by tapping the Add Medicaid Health Plan Card button. 

• If you see Accept and Decline buttons, you have an older LA Wallet app version and will need to update your app in the Apple App Store or Google Play Store.

3. Enter either your Social Security Number or Medicaid CCN to search for your Medicaid health card.

• Your first name, last name, and date of birth will automatically be provided from the license or ID linked in LA Wallet.
• Once your information is entered, tap Submit to search for your Louisiana Medicaid Health Card.
