Key Information

• You must link your Medicaid card FIRST before you can link a household member's Medicaid card.
• LA Wallet supports Medicaid cards for United Healthcare, Healthy Blue, Louisiana Healthcare Connections, AmeriHealth, Humana, Aetna, and Fee-for-Service health plans.
  • LA Wallet does not support Medicare cards. 
• You must be listed as the Head of Household on the household member's Medicaid file with the Louisiana Department of Health (LDH).
• If you can't find your dependent's Medicaid Health Plan Card, email help@lawallet.com with the following details to escalate the issue to LDH for further help.
  1. Dependent's Name
  2. Dependent's Date of Birth
  3. Dependent's CCN
  4. Dependent's Type of Louisiana Medicaid
  5. Your Name
  6. Your Date of Birth
  7. Specific available days and times for LDH troubleshooting

Instructions

1. After linking your Medicaid health plan card, you can link a household member's card by tapping the Medicaid Health Plan Card on the Home tab.

2. Next, tap the 3 lines to the top right of the Medicaid Health Plan Card screen and select Add Household Member

3. You will then fill out the boxes with the household member's information, and tap Submit.

• You must be listed as the Head of Household on the household member's Medicaid file with the Louisiana Department of Health.

4. Once their information is found, their card will be added to your account.
