• Your information is sent and the issue will be handled directly by the Louisiana Department of Health (LDH). 

Instructions

1. Tap the Menu tab, scroll under Louisiana State Services, and select the Vaccination Card Assistance option.

2. You will be asked if you need assistance with your health card or a child/dependent's. You will need to tap My Health Card.

3. You will be asked if your vaccinations were given outside the state of Louisiana. Tap Yes.

4. Next, fill out a form for additional assistance. Once all boxes with a red asterisk(*) have been filled out, the user can tap Next to go to the next page.

• You can provide a phone number for LDH to contact you about your vaccinations.

5. The next page will require you to Select Reason for Escalation and submit a picture then tap Submit for a ticket to be escalated to LDH.

6. LDH will handle contacting and assisting you from here on out. You will need to reply directly to any email or phone communication that you receive from LDH.
