Key Information

• LA Wallet now displays all completed vaccinations in Louisiana for dependents.
  • Anyone 18 and over will need to create their own LA Wallet account to view their vaccinations.
• You do not have to link your vaccinations to link a dependent's vaccination record. 

Instructions

1. After linking your driver's license or state ID, tap the Vaccination Card on the Home tab.

2. If your vaccinations are not linked, you will see a Terms and Conditions page, which you can accept by tapping the Add Dependent Vaccination Card button.

3. You will need to fill out the dependent's First Name, Last Name, Suffix (if applicable), Date of Birth, and Zip Code then tap Add Dependent.

4. Once the dependent's information is found, you will automatically be able to see their vaccinations.

• Tap their name to view a complete list of their vaccinations.
