If you are missing a vaccination shot that was received from a Louisiana healthcare provider, try tapping the “Refresh” button on the SMART Health Card and see if that gets your new or missing vaccine information showing.
If not found, please open up the Menu tab in the LA Wallet app and then click on Health Card Assistance. That will get you a ticket opened in a support channel with the Louisiana Department of Health who will be able to assist further with any COVID-19 related issues. They will assist you further with this issue through email or you can add a callback number when you fill out the Health Card Assistance ticket in the app.
