Please contact your vaccination provider and have them check your information in the Louisiana Department of Health's LINKS immunization database. If your name, date of birth, or address is not listed exactly as it is on your driver's license/ID, your information won't be available in LA Wallet.

If this has already been done, please submit a Health Card Assistance ticket under the Menu tab of the LA Wallet app so we can get this escalated to the Louisiana Department of Health for you. They will assist you further with this issue through email or you can add a callback number when you fill out the Health Card Assistance ticket in the app.
